Elevated Risk of Uncontrolled Bleeding with Pradaxa

In the first quarter of 2011, the U.S. Food and Drug Administration received more than 500 reports of life-threatening bleeding associated with the drug Pradaxa, a blood thinner used primarily with patients who have non-valvular atrial fibrillation, a heart irregularity that greatly elevates the risk of stroke.

In its quarterly report, the Institute of Safe Medication Practices (ISMP), which reviews reports filed through the FDA MedWatch program, identified Pradaxa as being a more problematic medication than all others currently being monitored. This is largely due to the fact that there is no known treatment for Pradaxa-related bleeding.

Overview of Pradaxa

Pradaxa, a product of Boehringer Ingelheim Pharmaceuticals, was released in October 2010. In the first three months the drug was on the market, it received more adverse event reports through MedWatch than 98.7 percent of all other drugs. Critics are now raising questions about the validity of the clinical trials conducted before the product’s approval.

Pradaxa has been marketed as a replacement for the older medication Warfarin. In theory, users of Pradaxa require less medical supervision, but in cases of bleeding from Warfarin use, the problem can be treated with Vitamin K. The lack of a viable remedy for Pradaxa-related bleeding automatically increases the risk profile of the drug.

Additional Pradaxa Dangers

The prevalence of hemorrhages associated with Pradaxa use is especially marked in the elderly and those patients with existing kidney insufficiency. In addition, there are reports of both pulmonary embolism and deep vein thrombosis associated with blood clots that Pradaxa failed to address.

The FDA began a safety review of Pradaxa beginning in December 2011, some 14 months after the drug originally came on the market. The official investigation was launched approximately one month after the drug’s manufacturer began its own inquiry into Pradaxa-related deaths.

Pradaxa Issues Explained

In October 2010, the U.S. Food and Drug Administration approved a prescription medication manufactured by Boehringer Ingelheim Pharmaceuticals under the brand name Pradaxa. (The chemical name for the medication is dabigatran etexilate mesylate.)

Pradaxa is a blood thinner used specifically in patients who suffer from non-valvular atrial fibrillation, a condition in which the organ’s two upper chambers beat irregularly or “flutter.” It places patients at an increased risk for stroke, which Pradaxa, as a direct thrombin inhibitor helps to prevent by lowering the chance of clotting.

Internal Bleeding with No Viable Treatment

Within three months of its introduction to the market in the U.S., Pradaxa was prescribed to about 86,000 individuals, with the standard caution of this class of drugs that internal bleeding can occur. Pradaxa was judged to be safer than the older, more standard Warfarin, which has a history of adverse food reactions. When Warfarin causes bleeding, however, Vitamin K will treat the issue. There is, at present, no viable treatment for bleeding in Pradaxa patients.

Other side effects associated with Pradaxa in addition to excessive internal bleeding include:

  1. excessive external bleeding,
  2. pain and swelling in the joints,
  3. dizziness,
  4. nausea and other gastrointestinal distresses.

FDA Review of Pradaxa

On December 7, 2011, the FDA issued a Drug Safety Communication for Pradaxa, saying the medication is under review in light of reports of fatal bleeding, but the source of those reports was not disclosed.
